MEDICAL BENEFITS REMINDER

Additions/Deletions From Health Benefits

Getting married??

Adding a child to your coverage??

Getting Divorced??

Changes in dependent coverage MUST be made within

60 days of the "event".

If you need to ADD A SPOUSE OR NEWBORN TO YOUR INSURANCE COVERAGE, this must be done within 60 DAYS from the MARRIAGE OR BIRTH OF THE CHILD.

This cannot be done at any time. If you do not add the dependent within the 60-day time frame, you will have to wait until the following open enrollment period.

Simply bring a copy of your marriage certificate or birth certificate of your child to the Health Benefits Department at 33 Church Street and fill out the application. In doing so, you will avoid the unfortunate experience of some people who were forced to wait for insurance to be effective for their dependents, as a result of not completing this task.

If you DIVORCE AND MUST DELETE A FORMER SPOUSE FROM YOUR INSURANCE, this too must be done within 60 days of the finalization of the divorce. Failure to properly notify the District could result in your being required to repay the District for "excess" coverage.

This procedure is outlined in your State Health Benefits Plan booklet and has been in effect since July 1,1992, when the District went back to this insurance plan. If there are any questions about this procedure, please call P.E.A. at (973) 790&emdash;3311, or the District Health Benefits Office at (973) 321- 0745.

And Remember … retain a dated copy for your records of any notification made to the District in the above matters!!!
